Waivers

Some activities and programs require waiver forms for your child to participate. Fill out the waiver forms that correspond to your child's program or activities.

Horseback Riding

All campers need the general SCUBA diving form.
If you answer "yes" to ANY questions in the medical section, you must also fill out the SCUBA Medical Statement.
